Hoarding Ice-Breaker Form
Is clutter, disorganisation, or hoarding are affecting your health?
Use the Hoarding Ice-Breaker Form.
Because your health comes before the clutter.
There may be several reasons why you’re feeling unwell, so try not to assume there’s only one cause or one solution.
Once you’ve completed the Hoarding Ice-Breaker Form, the person you share it with, whether it’s your GP or another professional, should ask a range of questions to help them understand what might be contributing to your symptoms and explore ways to support your wellbeing.
They may suggest involving others to provide practical help for you or your family. If that feels difficult or unwelcome, say so clearly rather than simply agreeing.
To make the most of your appointment, it can help to prepare in advance. Think about the questions they might ask about the areas you’ve ticked or circled, and jot down what you’d like to say in case you feel flustered on the day.
Your experiences may differ, the most important thing is to be open and honest.
